Faxing us instead?

Prefer paper? These forms are built to survive a fax: black and white, one character per box, and a barcode on each form that identifies it.

For a clean fax

  1. Print at 100%, in black and white.
    Use Letter paper and choose “Actual size” so the boxes and barcode keep their proportions.
  2. One character per box.
    Print in capital letters and numbers with a dark pen, and stay inside the boxes.
  3. Fill in every required field.
    Fields marked with an asterisk (*) are required.
  4. Fax it to 701.499.7404.
    Shipment forms must have your Customs invoice(s) attached, with the PAPS or CBSA code/Shipment Control Number.
